*All new students and 11th grade students must have a current physical on file with the nurse  prior to the first day of school

This physical with immunization update is typically done after the last day of 9th grade and no later than the first day off 11th

Please contact the nurse if you are having difficulty obtaining the documentation from your provider

The CT Blue Health Assessment form also fulfills the physical requirements for athletics participation. Physicals must be updated every 13months for athletics participation. Forms can be found on the District Health page or through the Athletics web page
